Is there a reason Vortex is left at level 1 ?
Dose it neagtively affected the build if its any level higher than 1 ?
Its damage is pretty low compared to Icestorm, so keeping it lvl1 saves little bit mana. Few leagues back they changed CwC and CwDT (and other autotrigger gems) so that when they trigger skill, it also costs skills normal manacost. This might mean you run out of mana sometimes if you don't have good manaregen and CwDT is proccing all the time.
But if you don't have manaproblems the level dosen't really matter. Taking the "10% of Damage taken Recouped as Mana" from Mana Mastery trivializes this and lets you run even maps with no mana regen easily.
Is it also because we really only use Vortex to proc culling vs. bosses? So it really doesn't matter how much damage it does, just that it hits.

With the harvest craft there is a possibility to roll 3 prefixes, so you would need to win 50/50 with annul to be able to continue, right?
well mb it was luck or smth...every time after reforge it was 1, max 2 pref...
checked in craft emulator...60-70% it was only 1 pref - infl, only one time 3 pref was after 30 times mb
so if you very unlucky you can get 3 pref after first reforge...but most time it's 1-2 pref after reforge. 1 of them infl... so i thinkg suff lock craft and reforge infl is best way to get % dmg per 15 int

I have the same layout and it works for me. Make sure Ball Lightning and CWDT are both level 1.

You have lvl20 CwDT in your setup. That means the CwDT is trying to cast the curses. You want low level (lvl1) CwDT so it triggers only Ball Lightning that then triggers curses.
Also you don't have Whispers of Doom, so you can have only 2 curses at time. This does not disable all of them, just leave the 2 curses that are applied last in effect. This is decided by the order of the gems in link, starting from top-left Frostbite first, Elemental Weakness second and Enfeeble third, so you end up missing frostbite.
